Geriatrics

Geriatric Medicine services are concerned with all aspects of health and wellbeing in older adults, on an outpatient and inpatient basis. The Department of Geriatric Medicine provides acute, day hospital, rehabilitation, community liaison and nursing home outreach services to patients from the local catchment area as well as clinical support and advice on the management of geriatric patients admitted on general medical call to the hospital and to the Acute Medical Unit.

Key Services

At UHK our Geriatric Medicine Consultants provide outpatient and inpatient services. They perform comprehensive geriatric assessments and develop treatment plans which may include rehabilitation, support and long term follow up. They link with day hospital, rehabilitation, community liaison and nursing home outreach services to provide care for patients from the local catchment area.

The Department is integrated with the Integrated Care Programme for Older Persons (ICPOP) with consultant sessions across both hospital and ICPOP sites. The Department is also supported by three advanced Nurse Practitioners in Frailty, Dementia and Orthogeriatrics.
